Madonna is terrified that ex-husband Sean Penn is chain-smoking and drinking himself into an early grave, sources tell the National Enquirer.
“It’s brutal for her to watch what Sean is doing to himself right now,” says an insider. “He seems to be in self-destruct mode — he acts like he’s bulletproof but it’s obvious his lifestyle is taking a huge toll on him.”

The One Battle After Another star, 65, looks years older than his age. His bad habits have the wellness-obsessed “Material Girl” singer, 67, fearful for his health.
“Madonna has tried to talk sense into him and get him to make some changes but she’s had no luck,” the insider says. “So now she’s started reaching out to other people in his inner circle to discuss options.”
“She says she wouldn’t be able to live with herself if something happened to him and she hadn’t tried to help.”

Madonna and Penn, who were married from 1985 to 1989, have remained close friends even as both moved on with other partners.
The mom of six would love to do some sort of intervention, but she knows she needs backup if she wants to help persuade the Mystic River star to change his ways.
“The problem is that Sean hasn’t really hit any sort of rock bottom,” the insider says. “No matter what anyone says he refuses to slow down.”

According to the insider, Sean’s been able to maintain the same pace of boozing and smoking for years and doesn’t see any issue with continuing, so the odds of him accepting any sort of help are slim to none.
“There’s a sense that he’s going to need something drastic to happen before he pulls out of this,” says an insider. “It’s left a lot of his loved ones feeling pretty sad and helpless, Madonna included.”
Even after all these years, the Rock & Roll Hall of Fame inductee is still concerned about Sean’s well-being, even admitting in her documentary Truth or Dare a couple of years after their separation that he was the love of her life.
